AGREEMENT OF RELEASE AND WAIVER OF LIABILITY

It is my responsibility to inform the facilitator of any limitations before the session begins.

Please read the following and ask if you have any questions:

I understand that Reiki is a gentle, non-invasive complementary wellness practice intended to support relaxation, stress reduction, and overall well-being. Reiki practitioners do not diagnose medical conditions, prescribe medications, perform medical treatment, or claim to cure or prevent any disease or illness.I understand that Reiki is not a substitute for medical care, mental health care, psychological counseling, or treatment provided by licensed healthcare professionals. I agree to seek appropriate medical advice for any physical or mental health concerns and to continue any prescribed treatment unless directed otherwise by my healthcare provider. I understand that Reiki sessions may involve: Light, stationary touch with my permission, hands held just above the body without physical contact, placement of crystals on or around the body, clothing, massage table, or surrounding space. I understand that I may request that no physical touch or crystal placement be used during my session.

Sound therapy (including sound baths) and energy work are not substitute for medical attention, examination, diagnosis or treatment. Sound therapy and energy work are not recommended and not safe under certain medical conditions. By signing, I affirm that a licensed physician has verified my good health and physical condition to participate in such programs. In addition, I will make the practitioner aware of any medical conditions or physical limitations before class. If I am pregnant, become pregnant or I am post-natal or post-surgical, my signature verifies that I have my physician's approval to participate. I also affirm that I alone am responsible to decide whether to practice and participation is at my own risk; in consideration in participation in sound therapy/healing and energy work. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against Ohuru Yoga, its owners, officers, employees, and instructors. It is my choice to receive a vibrational sound therapy and I understand that the practitioner will be using gentle sound and vibration during the sessions on/around me. Sound therapy/healing or energy work is a method of relaxation and stress reduction. I understand that practitioners do not diagnose illness, disease, or physical or mental disorders, nor do they prescribe medical treatments or pharmaceuticals. I acknowledge that these sessions are not a substitute for medical examination or diagnosis, and that is recommended I see a primary health care provider for those services.

I, the undersigned, understand that breathwork involves conscious, controlled breathing techniques that may result in physical and emotional responses. I understand that these techniques can include sensations such as tingling, changes in body temperature, emotional release, or light-headedness, and are intended to promote relaxation, awareness, and personal growth. I confirm that I do not have any medical conditions, including but not limited to: cardiovascular issues, severe hypertension, epilepsy, bipolar disorder, pregnancy, schizophrenia, or recent surgery, that would make participation in breath work inadvisable. If I do have a condition, I affirm that I have consulted with a licensed medical provider and received approval to participate.

Information shared during sessions will be treated as confidential except where disclosure is required by law or when there is an imminent risk of harm to myself or others.

I, my heirs or legal representatives forever release, waive, discharge and covenant not to sue any Released Party for any Claim caused by any negligence or other acts of a Released Party.

I have read and fully understand and agree to the above terms of this Agreement and Release of Waiver of Liability.

I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional

release of all liability to the greatest extent allowed by law in the State of California.
